PROMOTING HEALTH, WELLNESS, AND INDEPENDENCE THROUGH ACCESS TO BALANCED NUTRITIONAL SUPPORT
Meal & Nutrition Services in Wyoming support individuals who are unable to safely prepare or access adequate meals due to disability, aging, or chronic health conditions. These services are essential for maintaining health, managing medical conditions, and supporting independent living in the home or community setting.
Meal and nutrition support is available through Wyoming’s Medicaid Home and Community-Based Services (HCBS) Waivers, including the Community Choices Waiver (CCW) for seniors and adults with physical disabilities and, in some cases, through the Comprehensive and Supports Waivers for individuals with developmental disabilities. Services must be included in the participant’s Individualized Plan of Care (IPC) and tied to functional needs.

Agency: Wyoming Department of Health – Division of Healthcare Financing & Aging Division
Role: Defines meal service policies, approves providers, and monitors nutrition-related outcomes under Medicaid waiver programs.

Agency: Case Management Agencies / Public Health Nursing (PHN)
Role: Determines individual meal support needs and integrates them into the participant’s IPC.
2. MEAL & NUTRITION SERVICES OVERVIEW
Meal and nutrition services ensure participants have access to healthy, appropriate meals to support dietary needs and promote safety at home. Services vary based on waiver and participant condition.
Covered services may include:
Home-delivered meals (e.g., frozen or fresh, medically tailored)
Congregate meals (at senior centers or adult day programs)
Grocery or meal kit delivery (when preparing food independently is a challenge)
Specialized diets (diabetic, low-sodium, allergen-free, etc.)
Nutritional counseling provided by a licensed dietitian (if authorized)
Meal preparation in the home (if part of personal care or homemaker services)
Services must align with dietary needs, be cost-effective, and avoid duplication with other services like SNAP or in-home supports unless medically justified.
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register with the Wyoming Secretary of State
Obtain Federal EIN and Type 2 NPI
Apply for Medicaid waiver provider enrollment for meal services
Have or partner with a licensed kitchen or meal vendor that meets food safety standards
Develop a Meal Services Policy & Procedure Manual

If providing nutritional counseling, a licensed registered dietitian must be employed or contracted.
4. WYOMING PROVIDER ENROLLMENT PROCESS
