2. HOME MODIFICATION SERVICE OVERVIEW
Home Modification Services involve permanent or semi-permanent changes to a participant’s residence to remove barriers and enhance independence. All modifications must be directly related to the individual’s disability, pre-approved by DDDS, and documented in the ISP.
Approved providers may deliver:
- Installation of wheelchair ramps and stair lifts
- Roll-in showers, grab bars, and raised toilets
- Widening of doorways and hallways
- Lowering counters, sinks, and light switches

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
