2. HOME MODIFICATION SERVICE OVERVIEW

Home Modification Services involve permanent or semi-permanent changes to a participant’s residence to remove barriers and enhance independence. All modifications must be directly related to the individual’s disability, pre-approved by DDDS, and documented in the ISP.

Approved providers may deliver:

• Installation of wheelchair ramps and stair lifts
• Roll-in showers, grab bars, and raised toilets
• Widening of doorways and hallways
• Lowering counters, sinks, and light switches
• Installation of visual/auditory alert systems
• Modified flooring and non-slip surfaces
• Pre-project assessments, construction, and post-installation documentation

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

• Register your business with the Delaware Division of Corporations
• Obtain EIN from the IRS and NPI (Type 2)
• Hold a valid Delaware contractor’s license or subcontract licensed professionals
• Apply as a Home Modification Services Provider under DDDS
• Carry general liability and product liability insurance
• Maintain HIPAA-compliant documentation systems
• Use ADA-compliant building standards and waiver-approved materials

4. PROVIDER ENROLLMENT PROCESS

Step 1: Register your business and obtain NPI
Step 2: Submit a provider application to DDDS for Home Modification Services

Include contractor license, proof of insurance, and sample project protocols

Step 3: Enroll with Delaware Medicaid (DMAP) if billing directly
Step 4: Collaborate with support coordinators and therapists for referrals, evaluations, and ISP approvals
Step 5: Begin project assessments, execute modifications, and submit documentation of completion

6. STAFFING REQUIREMENTS

Role: Licensed Contractor / Project Manager
Requirements:

• Valid Delaware contractor’s license
• Experience with ADA-compliant modifications
• Background checks and general liability coverage

Role: Accessibility Evaluator / Consultant (optional)
Requirements:

• Knowledge of home assessments for individuals with disabilities
• Familiarity with Medicaid waiver and ISP coordination

Training Requirements for All Staff/Subcontractors:

• Job site safety and ADA compliance
• Documentation and waiver billing procedures
• HIPAA and client confidentiality
• Emergency response procedures

7. MEDICAID WAIVER SERVICES

Home Modification Services are reimbursed under:

• Delaware Lifespan Waiver (administered by DDDS)

Approved providers may deliver:

• Environmental assessments
• Structural modification implementation
• Training on new features (as needed)
• Documentation of project scope, cost, and completion
