Adult Day Health Services in Connecticut 


1. Program Definition and Services 

Adult Day Health Services (ADHS) in Connecticut provide comprehensive programs designed to meet the needs of cognitively and physically impaired adults. Services include: 

  • Socialization and supervision 
  • Personal care assistance 
  • Health monitoring 
  • Rehabilitative services (in medical model) 
  • Nutritional services 
  • Structured activities 
  • Connecticut offers two models of ADHS: 
  • Social model: Provides supervision and activities for those not needing extensive personal care and medical monitoring 
  • Medical model: Offers more extensive personal care, medical monitoring, and rehabilitative services 

 

2. Regulations 

ADHS are governed by Sections 17b-342-1 to 17b-342-5 of the Regulations of Connecticut State Agencies 

 

3. Licensing or Certification 

While Connecticut does not license ADHS centers, providers must be certified by the Connecticut Association of Adult Day Centers to receive state reimbursement.

 

4. Responsible State Agency 

The Department of Social Services (DSS) oversees ADHS as part of the Connecticut Home Care Program for Elders (CHCPE)  

 

5. Application Process 

Individuals can apply for ADHS through the CHCPE: 

  • By submitting a paper application (W-1LTSS form) to the appropriate Long-Term Services and Supports Application Center 

 

6. Required Documentation 

Applicants should provide: 

  • Proof of income and assets 
  • Medical records demonstrating the need for services 
  • Social Security card 
  • Medicare card (if applicable) 
  • Other relevant documentation as requested 

 

7. Timeline for Approval 

The application process can take up to 3 months from submission to determination 

 

8. Pre-Application Process 

No specific pre-application process is mentioned, but applicants should ensure they meet eligibility criteria before applying. 

 

9. Pre-Application Training 

No pre-application training is required for ADHS recipients. 

 

10. Additional Notes 

  • As of February 2025, there are more than 40 adult day centers located in Connecticut offering door-to-door transportation from neighboring communities 
  • ADHS providers must meet all applicable state and local requirements, including zoning, sanitation, fire, and safety requirements 
  • Providers serving Medicaid waiver participants can serve individuals who meet the state's nursing home level of care criteria
