3. Licensing or Certification
Providers must register as an established medical or assistive technology business with the NJ Division of Consumer Affairs. Clinical or assessment staff must hold state licensure (e.g., PT under N.J.A.C. 13:39A, OT under N.J.A.C. 13:44K, ST under N.J.A.C. 13:44C) or be certified as an Assistive Technology Professional (ATP) through RESNA.
4. Responsible State Agency
The New Jersey Department of Human Services (DHS), the Division of Medical Assistance and Health Services (DMAHS), and the Division of Developmental Disabilities (DDD) collectively handle program compliance, readiness reviews, and enrollment authorizations.
5. Application Process
Applicants must first complete and submit the DDD Statement of Intent (Form FD-459 DDD/SOI) to verify service capabilities. Once cleared, the complete Medicaid enrollment packet (including Form FD-23B) is processed through the New Jersey Medicaid Provider Enrollment Portal (NJMMIS).

- Adaptive equipment purchases through waiver programs are capped at $2,500 per year per participant unless specific prior approval is granted by DDD.
- Items covered under the standard Medicaid State Plan cannot be double-billed under the HCBS waiver service.
