2. ADAPTIVE EQUIPMENT SERVICE OVERVIEW

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

• Register business with the Arkansas Secretary of State
• Obtain EIN from the IRS and NPI (Type 2)
