  • Health Oversight & Community Access: Routine medication administration under registered nurse delegation workflows, behavioral support integration, coordination of community volunteerism or social events, and dedicated transit to authorized habilitation activities

Settings accommodating 4 or more clients simultaneously require full Residential Support Licensure for each specific physical address.

 

4. Responsible State Agency

The Utah Department of Health and Human Services (DHHS), through the Division of Services for People with Disabilities (DSPD), administers the waiver contracting system and the MySupport case monitoring framework. All physical site health/safety rules, corporate licensure tracks, and background clearances are processed by the DHHS Division of Licensing and Background Checks (DLBC).

 

5. Application Process

  • Register the business entity with the Utah Department of Commerce, Division of Corporations
  • Submit the corporate profile and fingerprint-based background clearance applications via the DLBC Provider Portal
  • Enroll the corporation as an active waiver provider within the state's Medicaid PRISM portal

The combined processing pipeline spanning property physical health inspections, DLBC background screening, PRISM Medicaid activation, and final DSPD electronic contract validation typically spans 3 to 5 months.

 

8. Pre-Application Process

Prospective providers must form an LLC or Corporation with the Utah Division of Corporations, secure an EIN, and obtain a corporate Type 2 National Provider Identifier (NPI) mapped to residential care, intellectual disability support, or specialized community living taxonomy paths.

 

9. Pre-Application Training

The state hosts mandatory administrative and compliance training modules online.   • Any worker dispensing or distributing medications must undergo documented Medication Administration Training and operate under the written supervision and delegation of a licensed Utah Registered Nurse (RN)
  • Providers must enforce the mandatory Abuse, Neglect, and Exploitation Prevention (ANEP) standards, ensuring all workforce members file reports within 24 hours of any suspected critical incident to pass post-payment state Medicaid reviews
