Skilled Nursing Services Pennsylvania

Become a Skilled Nursing Services Agency Provider in Pennsylvania

1. Program Definition and Services

Skilled Nursing in Pennsylvania is defined as a program that provides medically necessary clinical care delivered by licensed nurses to individuals with complex health needs at home. Services include:

  • Medication administration, injections, and IV therapy
  • Advanced wound care and sterile dressing changes
  • Chronic condition monitoring and clinical assessments
  • Post-acute care following hospital discharge or surgery
  • Care plan development under direct physician oversight
  • Specialized medical task training for family caregivers

2. Regulations

The program is governed by 28 Pa. Code Chapter 601 (Home Health Care Agencies), 55 Pa. Code Chapter 52 (OLTL Home and Community-Based Services), and federal Medicare/Medicaid Conditions of Participation.

3. Licensing or Certification

Providers operating a home health model must obtain a Home Health Agency License from the Pennsylvania Department of Health (DOH). Medicare certification is also required prior to Medicaid waiver system validation.

4. Responsible State Agency

The Pennsylvania Department of Health (DOH) oversees agency licensing. The Department of Human Services (DHS), specifically the Office of Long-Term Living (OLTL) and Office of Developmental Programs (ODP), manages the waiver programs.

5. Application Process

Providers must first pass a DOH licensing inspection and secure Medicare certification. Approved agencies then complete electronic enrollment via the PROMISe™ Medicaid Portal under Provider Type 05 (Home Health Agency) and execute network contracts with regional Community HealthChoices (CHC) MCOs.

6. Required Documentation

While specific requirements vary by clinical tier, providers must typically provide:

  • Pennsylvania Business Registration (PennFile)
  • IRS EIN letter and Type 2 Organizational NPI
  • Approved PA DOH Home Health Agency License and Medicare validation
  • Certificates of active Medical Malpractice, Workers' Compensation, and General Liability insurance
  • Clinical Policy Manual covering plans of care (Form 485), infection control, and charting standards

7. Timeline for Approval

Navigating the sequential steps of state DOH licensing, federal Medicare certification, and PROMISe/MCO provider enrollment takes an average of 4 to 6 months.

8. Pre-Application Process

Before starting portal applications, register your business structure with the PA Department of State, obtain a federal EIN, secure a dedicated physical commercial office space for confidential medical records, and register for a Type 2 NPI.

9. Pre-Application Training

The agency Administrator must meet state experience and supervisory guidelines. All frontline nurses must hold an active Pennsylvania Nursing License (RN or LPN). Staff must clear Act 169 background checks, pass FBI fingerprinting, complete negative TB testing, and finish mandatory HIPAA and infection control modules.

10. Additional Notes

  • Skilled Nursing can be funded through the standard Medicaid State Plan, the CHC Waiver, or ODP waivers depending on participant eligibility
  • All clinical services require a direct prescription and a signed plan of care from the individual's primary physician or nurse practitioner
  • LPN direct care shifts require documented oversight and periodic home case evaluations conducted by an agency Registered Nurse (RN)
  • The primary goal of the program is to stabilize chronic illnesses and support medical recovery to safely prevent nursing home placement
  • Approved skilled nursing care is provided at no cost to authorized Medicaid participants who meet medical necessity guidelines
  • Nursing charts, physician orders, and precise visit timestamps must be maintained digitally for ongoing state audit readiness
