8. Pre-Application Process
Prospective providers must form an LLC or Corporation with the Wisconsin DFI, secure an EIN, and obtain a corporate Type 2 NPI. Agencies must also establish a vendor or supplier network with local furniture warehouses, appliance distributors, utility firms, and property management companies to ensure rapid household setup capability.
9. Pre-Application Training

- Providers must build and securely store a dedicated master transition archive for every participant, combining the initial institutional discharge authorization, line-item furniture receipts, municipal security deposit confirmations, and signed move-in safety verifications to satisfy retroactive state financial audits
