3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Obtain EIN from the IRS and NPI (Type 2)
  • Enroll as a Medicaid Waiver Meal and Nutrition provider via the New Hampshire Medicaid Provider Enrollment Portal
