Supported Living Services in Minnesota assist individuals with disabilities to live independently within their communities. These services offer personalized care, supervision, and skill-building support to enhance daily living and social integration. Supported Living Services are available under Minnesota’s Home and Community-Based Services (HCBS) Waivers.
1. GOVERNING AGENCIES

Role: Administers Medicaid programs, including supported living under HCBS Waivers

Role: Licenses and regulates supported living facilities and care providers
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Provides federal oversight and ensures waiver compliance
2. SUPPORTED LIVING SERVICE OVERVIEW
Supported Living Services focus on helping individuals live as independently as possible while providing the necessary support to ensure safety and well-being. Services are tailored according to the Individual Service Plan (ISP) or Person-Centered Plan (PCP) and may include:
- Personal care and assistance with activities of daily living (ADLs)
- Health monitoring and medication management
- Skills training for independent living (e.g., cooking, budgeting)
- Community integration and social support
- Transportation to appointments and community events
- Safety supervision and crisis intervention
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
- Register business with the Minnesota Secretary of State
- Obtain EIN from the IRS and NPI (Type 2)
- Enroll as a Medicaid provider with the Minnesota Department of Human Services
- Obtain a Comprehensive Home Care License from the Minnesota Department of Health
- Maintain liability insurance and staff training documentation
4. MEDICAID PROVIDER ENROLLMENT PROCESS

- Contact the Minnesota Department of Human Services to understand supported living provider requirements
Step 2: Application Submission
- Submit business registration, comprehensive home care license, liability insurance, and service descriptions

- Background check and TB clearance
Training Requirements:
- Person-centered planning and individual rights

7. MEDICAID WAIVER SERVICES
Supported Living Services in Minnesota are available through:
