Provider Requirements
- Must be licensed annually by the Alabama Board of Home Medical Equipment Services (HME)
- Need to obtain a $50,000 surety bond per National Provider Identifier (NPI)
- Required to have liability insurance with a minimum limit of $300,000
- Must enroll with the Alabama Medicaid Agency
