3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
- Register business with the Louisiana Secretary of State
- Obtain IRS EIN and Type 2 NPI
- Enroll with Louisiana Medicaid as a transportation provider via the Medicaid Provider Enrollment Portal
