3. Licensing or Certification

Provider agencies must register as a corporate entity and hold an active Home Improvement Contractor (HIC) Registration issued by the New Jersey Division of Consumer Affairs (NJHIC#). Any trade-specific sub-work (such as electrical modifications for lifts or specialized plumbing rerouting for roll-in showers) must be performed by professionals holding active trade-specific licenses (Licensed Master Plumber or Licensed Electrical Contractor) from their respective New Jersey state boards.

 

4. Responsible State Agency

The New Jersey Department of Human Services (DHS) specifically the Division of Developmental Disabilities (DDD) and the Division of Aging Services (DoAS), in partnership with the New Jersey Division of Consumer Affairs, acts as the primary state authority. 

 

5. Application Process

The provider enrollment sequence is managed online via the New Jersey Medicaid Provider Enrollment Portal (NJMMIS.COM). Prospective agencies must first clear state consumer affairs registration criteria and upload validation of active structural credentials. 

 

6. Required Documentation

  • SOS Certificate of Formation / Articles of Organization filed with the New Jersey Division of Revenue
  • Federal Employer Identification Number (EIN) and an active Type 2 National Provider Identifier (NPI)
  • A valid New Jersey Home Improvement Contractor (HIC) Registration certificate
  • Specific trade license verifications (Electrical/Plumbing) for in-house staff or primary subcontractors

7. Timeline for Approval

Typically 90 to 120 days from the receipt of a complete application packet, depending on state contractor registration verification, administrative review queues, and portal credentialing lags.

 

8. Pre-Application Process

Prospective providers must complete specific foundation tasks before applying. They must establish their legal business entity with the New Jersey Division of Revenue, submit their structural application to the Division of Consumer Affairs to secure an active HIC number, configure their federal EIN/NPI registry status, and establish a designated commercial office space or staging yard meeting municipal zoning rules.

The state requires administrative, compliance, and billing training sessions to be completed. Program Directors and lead accessibility coordinators must clear specialized Medicaid fraud modules, Electronic Visit Verification (EVV) updates where applicable, and HCBS Settings Rule documentation layouts before project authorizations can be issued.

 

10. Additional Notes

  • To comply with federal and state waiver rules, modifications are restricted to a participant's private residence (or a rental property with written landlord approval). Modifications cannot be made to common areas of apartment complexes or facilities owned/leased by waiver service providers
  • Adaptations that add to the total square footage of a home or are considered of general utility (such as carpeting, roof repairs, or central air conditioning) are strictly excluded from waiver funding
  • All modification jobs must comply with local municipal building codes, require proper local permits, and successfully pass local code enforcement inspections prior to final Medicaid reimbursement
  • All administrative directors, licensed contractors, field installers, and project estimators must clear mandatory fingerprint background checks via the state-approved IdentoGO network
