2. MEAL & NUTRITION SERVICE OVERVIEW

Approved providers may deliver:

Home-delivered meals (daily or weekly delivery)

Congregate meals (if offered at program locations)

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

Register your business with the Arizona Corporation Commission

Obtain EIN from the IRS and NPI (Type 2)

Apply through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)

Meet local food safety and handling regulations (Maricopa County or local jurisdiction)

4. DDD QUALIFIED VENDOR ENROLLMENT PROCESS

2. ASSISTED LIVING FACILITY SERVICE OVERVIEW

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

Secure liability insurance and worker’s compensation coverage
