
SUPPORTING INDEPENDENCE AND SAFETY THROUGH ACCESS TO MEDICALLY NECESSARY EQUIPMENT AND SUPPLIES
Specialized Medical Equipment Services in Alaska provide individuals with disabilities and chronic conditions the equipment and supplies they need to function more independently and safely in their homes and communities. These services are offered under Alaska’s Home and Community-Based Services (HCBS) Medicaid Waiver programs and must be prescribed based on medical necessity and included in the individual’s person-centered service plan.
1. GOVERNING AGENCIES
Agency: Alaska Department of Health – Division of Senior and Disabilities Services (SDS)
Role: Approves providers, authorizes equipment requests, and ensures quality and appropriateness of equipment and services
Agency: Alaska Medicaid (under the Department of Health)
Role: Manages provider enrollment, claim reimbursement, and Medicaid compliance for medical equipment services
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Provides federal oversight and ensures that waiver-funded medical equipment aligns with HCBS requirements
2. SPECIALIZED MEDICAL EQUIPMENT SERVICE OVERVIEW
Specialized Medical Equipment Services provide devices, supplies, and equipment that enable individuals to increase their independence, ensure safety, or substitute for human assistance. These items must be directly related to the participant’s functional limitations and must not be available under standard Medicaid durable medical equipment (DME) benefits.
Approved providers may deliver:
- Mobility devices (e.g., manual/power wheelchairs, walkers)
- Lifts and transfer equipment (e.g., Hoyer lifts, ceiling tracks)
- Adaptive communication or vision/hearing devices
- Specialized beds, positioning equipment, or pressure-relief mattresses
- Medical alert systems, monitoring equipment, or environmental controls
- Repairs and maintenance of approved equipment
- Training on equipment use and care
All equipment and supplies must be pre-approved by SDS and supported by professional documentation (e.g., physician’s order, therapist recommendation).
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
- Register business with the Alaska Division of Corporations, Business and Professional Licensing
- Obtain EIN from the IRS and NPI (Type 2)
- Hold an Alaska Durable Medical Equipment (DME) license, if required
- Secure a physical location or distribution base in Alaska
- Maintain inventory management and secure storage systems
- Carry general liability and product liability insurance
- Establish service tracking, delivery, and maintenance protocols
4. SDS PROVIDER ENROLLMENT PROCESS
Initial Interest and Application:
- Request the Specialized Medical Equipment Services provider application from SDS
- Attend a provider orientation or consultation with SDS (optional but recommended)
Application and Submission:
- Submit a service plan including types of equipment offered, delivery methods, and quality assurance processes
- Include sample documentation: physician order templates, delivery logs, service tracking forms, and consent forms
- Provide business license, facility details, insurance documentation, and staff qualifications
Readiness and Compliance Review:
- Review of ordering, storage, and delivery systems
- Evaluation of staff training protocols and customer service systems
- Inspection of equipment handling, maintenance, and documentation processes
Approval & Medicaid Enrollment:
- After SDS approval, enroll through Alaska Medicaid MMIS as a provider of Specialized Medical Equipment
- Activate relevant billing codes under applicable HCBS waivers
- Finalize Medicaid provider agreement and electronic claims setup

6. STAFFING REQUIREMENTS
Role: Equipment Services Coordinator / Program Manager
Requirements: Experience in DME provision, logistics, or rehabilitation technology; knowledge of Medicaid processes
Role: Delivery & Setup Technician
Requirements: High school diploma or GED, technical experience in equipment assembly and maintenance, valid driver’s license
All staff must complete:
- SDS service delivery and HCBS waiver training
- Equipment safety, infection control, and maintenance training
- Documentation and confidentiality protocols
- Training on consumer interaction and use instruction
7. MEDICAID WAIVER SERVICES
The following Alaska Medicaid Waivers authorize Specialized Medical Equipment Services:
- Alaskans Living Independently (ALI) Waiver
- Adults with Physical and Developmental Disabilities (APDD) Waiver
- Individuals with Intellectual and Developmental Disabilities (IDD) Waiver
- Children with Complex Medical Conditions (CCMC) Waiver
Approved providers may deliver:
- Delivery and setup of medically necessary equipment
- Participant instruction and consent documentation
- Equipment maintenance, repair, and warranty tracking
- Coordination with prescribing professionals and service planners
