Medical supply services in Alabama provide essential equipment and products to individuals with medical needs, often through Medicaid coverage. 

Service Description 

Medical supply services include: 

  • Durable Medical Equipment (DME) 
  • Disposable medical supplies 
  • Incontinence products 
  • Oxygen and respiratory equipment 
  • Mobility aids 
  • Wound care supplies 

 

Provider Requirements 

  • Must be licensed annually by the Alabama Board of Home Medical Equipment Services (HME) 
  • Need to obtain a $50,000 surety bond per National Provider Identifier (NPI) 
  • Required to have liability insurance with a minimum limit of $300,000 
  • Must enroll with the Alabama Medicaid Agency 

 

Regulations 

  • Governed by Alabama Medicaid Agency regulations 
  • Must comply with Alabama statute 560-X-13-.02

 

Application Process 

  • Obtain city/county and state business licenses 
  • Purchase required insurance and surety bond 
  • Complete online application with the Board of Home Medical Equipment Services 
  • Pay applicable fees ($500 new location fee, $250 licensure fee) 
  • Pass physical location inspection 
  • File surety bond with Alabama Medicaid Agency 

 

Additional Notes 

  • Licenses expire on August 31 each year and must be renewed 
  • Providers supplying oxygen need a permit from the Alabama Board of Pharmacy 
  • As of February 2025, there are specific COVID-19 related updates for pharmacy providers
